We will require some basic information about the patient and their insurance coverage (this information is typically found on your insurance card). With this information we can provide you with coverage information that includes: number of visits per year, referrals, deductibles, co-payments and co--insurances.
Our administrative staff are always available to make appointments, answer questions about our services, describe monthly specials and help you purchase gift certificates either over the phone or via the internet.
New patients can schedule free consultations with our doctors or therapists. You can also request health intake forms that can be completed at home prior to your first visit at our office. These forms can be faxed or mailed to your home.
